How can I link a certain webpage to appear when a specific value is entered into a search form?

I'm using Adobe Dreamweaver CS5 (I'm somewhat new to using it, I began using it when I started in a web design class this spring) Anyways, I want to create a search form where the user can enter their zipcode into the search form and then submit that zipcode and depending on which zipcode they enter, it will link to a different page. I want to have it so that it will bring up a different page when each of the 3 specific zip codes are entered into the search form. Could someone who is good with web design offer me a little help with this please?
